Can divers develop joint problems over time?

Yes, this is called osteonecrosis and it is characterized by the degeneration of bone due to the obstruction of the terminal vessels of the bone's vascular supply.  In the case of diving this is probably due to gas emboli, brought on by frequent exposure to high pressures, inadequate decompression on ascent, and possibly by decompression sickness and poor treatment of the condition.
